Is there a way to create a report that shows ONLY completed items by Project?

You can go to your Done Tab on the Review Actions screen, search for the project in the Search filter and use the Print screen button to produce a report listing all the actions selected on the Tab. You can tailor the columns on the report. The help for this functionality is under the Review Actions screen help.

The only problem is that you cannot have the Notes included in the report.
